Without a diagnosis at the start of your sickness leave, all ‘may shift’.

Employer can start having doubts about the nature of your illness, and prepare a dismissal procedure because of decreased performance.

Without diagnosis, you do not know what kind of help you need.

Without diagnosis, you do not know on what skills you have to judge your coach or psychologist.

Returning to work without a) problem analysis (as in a document like this: PA – Problem Analysis UK) b) without burnout recovery coaching that includes ‘research’ on the causes of your burnout (the work context causes) without c) coaching on your coping d) removal of major stressors e) practical advice how to deal with the stressors, energy drainers and energy replenishers, is in a way ‘suicide’.

After the inital ‘freedom’ of 100% sickness leave you will feel better, but notice how huge your anxiety is to start again working. This anxiety is because you know you did not solve the problems that caused burnout. Burnout is totally different from ‘a flue’, it does not happen accidently and it does not cure from itself.

Returning to work after burnout recovery coaching

After you have almost completed your burnout recovery coaching, meaning that you:

  • now see what the causes of your burnout were
  • know what you want to do differently in future
  • are ensured main stressors are removed by employer
  • know far better how to cope with the stressors that ‘brought you down’
  • know far better how to replenish your energy reserve after work
  • sleep better

you can think about taking work up again. Months after the initial and first burnout sickness day.

The burnout time needed for recovery is always put under pressure. We see many attempts to pick up work after 2 months, fail again, and so forth….until maybe 12 times until 24 months of illness are over, and the contract is being dismantled. 2 months is far too short. EVEN WITH APPROPRIATE burnout recovery coaching burnout recovery requires an natural 3 months, at least 6 months before you can be fully operational again. It is very important that, when you pick up work again:

  • you feel social, emotional support from the employer (that is already written in the first burnout articel ever, Dr. Herbert Freudenberger, ‘Burn Out Staff’ in 1974, see Google Scholar)
  • you pick pieces of work that give you PLEASURE and ENERGY
  • you are not put in a rigid scheme to pick up work again, but that you can arrange your work organically – meaning: that you can listen to your emotion and physical signs, and stop working when you feel your energy level is too depleted again. ‘Going into the red zone’ of depleted energy makes sure you fall BACK into burnout. Avoid coming into the red zone for many months (no ‘deadlines’! Deadlines will not kill you but make you burnout again. You can only deal with deadlines again when fully recovered).

It is your employer (not company doctor!) that decides when you have to go back to work and what you should do.

Emapthy towards burnout from the employer is key, otherwise you will never recover in the job.

(in latter case: opt, with employer, for ‘tweede spoor’, meaning: burnout recovery coaching and after that: outplacement, towards a different employer and job).

 

Een burn-out diagnose is moeilijk op schrift te krijgen

Huisartsen willen vaak geen diagnose stellen om ‘geen ruzie’ te krijgen met bedrijfsartsen en Arboartsen. Helaas hebben Arbo- en bedrijfsartsen sinds de presentatie op 20 oktober 2014 door mr. Rik Schuurmans aan NVAB besloten al het werk gerelateerde te ontkennen, niet op schrift te zetten en alle richtlijnen genoemd bij beroepsziekten niet meer te volgen. Dit is uiteraard niet in de haak, ook juridisch niet:

  • het is te kwader trouw; de nationale ‘diagnosefraude’ of ‘diagnoseweigering’ zoals sommigen het noemen zou best door het OM vervolgd kunnen worden als ‘misdaad tegen de menselijkheid’ (vergelijk het geval waarin alle artsen zouden besloten ‘kanker’ niet meer te erkennen omdat het hen te weinig oplevert…).
  • het is een onrechtmatige daad; een arts dient zich zo gedragen als in het maatschappelijke verkeer van ’hem’ verwacht wordt, en dat is: welzijn van de patiënt op de eerste plaats, eed van Hippocrates etc.
  • artsen zijn verplicht de professionele standaard te volgen, BW art 7:453 en dat houdt in: het volgen van de richtlijnen Lesa/NVAB/NHG inclusief diagnostische tests
  • artsen zijn verplicht een diagnose te stellen conform de WGBO.
